Welcome to Health Risk Services Inc. “FORMS” page where you will be able to access a variety of Administrative, Enrolment and Claim Forms. Both Employers and Employees will find all of the necessary forms pertaining to their specific needs.

You will find below the various types of plans that are currently available through Health Risk Services Inc. Please determine the type of Benefit Plan that is applicable to you and your current coverage and enter that plan. There you will find a variety of forms that apply to that particular plan. Choose from the list of forms provided, print the form and complete in full.

If you are an employee of a Group Benefits Plan, please note that the Enrolment Form requires a signature of Authorization by your employer or administrator of the plan as well as your own. Please ensure that both signatures are applied prior to submitting your form to HRS Inc.

Once you have fully and accurately completed your forms, please submit them to Health Risk services Inc. by one of the following methods:

**Please note that we are unable to accept Health Spending/Flexible Spending Account Claims online.  These claims can be sent via fax, email and regular mail. **
